About Chronic Liver Disease(Non-Alcoholic Fatty Liver Disease)

Liver disease is an asymptomatic disease. Your 8-12% of the liver can perform the operations of 100% of the liver. That much robust the liver is. The liver has various functionalities. The most important function of the Liver is to manage your metabolism. Because metabolism supplies energy to various parts of the body, depending upon the demand, when the liver starts malfunctioning, the body’s energy demand is not met. This results in faster aging and more stress on the body. You tend to start developing negative thoughts, and you start getting more fatigued. Because the liver is so robust, your blood test liver function test often appears normal. Liver disease is a silent killer because neither any trace is seen in the blood tests, nor any alarming symptoms appear in your body until it is quite late. Non-alcoholic fatty liver disease is a silent killer. You do not doubt that you might have any problems with your liver because you do not drink. You also do not get to see any anomalies in your blood tests. Therefore this instrument is provided so that you can check your liver condition non-invasively.

Significance of This CLDQ Screening Instrument

This is a clinically validated instrument developed by Younossi et. al1Younossi, Z. M., et al. “Development of a disease specific questionnaire to measure health related quality of life in patients with chronic liver disease.” Gut 45.2 (1999): 295-300. This tool has been validated by several researchers in different geographic and demographic settings. Highly significant correlations were observed between the overall CLDQ score with SF-36 PCS (r=0.82, p<0.0001)2Chawla KS, Talwalkar JA, Keach JC, Malinchoc M, Lindor KD, Jorgensen R. Reliability and validity of the Chronic Liver Disease Questionnaire (CLDQ) in adults with non-alcoholic steatohepatitis (NASH). BMJ Open Gastroenterol. 2016 Mar 16;3(1):e000069. doi: 10.1136/bmjgast-2015-000069. PMID: 27110379; PMCID: PMC4838661.. Before this tool was developed, SF-36 was the questionnaire-based liver disease screening instrument. Thus CLDQ is a reliable and valid instrument. This instrument helps the overall population to:-

  1. Screen their liver and identify the current and future risk of liver disease.
  2. Those individuals with the higher risks can go for follow-up blood tests, and calculate their chances of advanced stage fibrosis from the Fib4 instrument as provided with this tool as a follow-up mechanism.
  3. If even Fib4 score comes at the higher side, then you can visit your Hepatologist, or get in touch with the Lyfas team.
  4. Your Hepatologist or Lyfas Hepatology team will advise you to do USG of liver.
  5. If the clinical team sees scars in the liver, then they will put you into a lifestyle modification.
  6. If you get in touch with the Lyfas team, we will not only treat your liver, but also help you with emotional healing, and help you improve your mental health that gets impaired due to liver disease. This holistic improvement of your overall life, will start healing your liver and you can save yourself from going to advanced-stage liver disease.
  7. You can continue to monitor your Liver scores in every 14 days, to see how you are improving.

How this Instrument Can Help The Clinicians

If you are a practicing clinician, then you can use this tool amongst your patients as a triage tool to determine the need for a Liver Biopsy. Liver Biopsy is an extremely painful and non-invasive process. Besides being less available, Biopsy also causes a significant mental trauma. If you are practicing in remote parts of the world, where blood tests and USG facilities are fewer, and Biopsy patients need to Visit a longer distance then you can use this tool for your clinical decision-making.

This Tool Can Be Used for Following Diagnosis, Screening, Triage, and Differential Diagnosis Purpose

  1. Nonalcoholic steatohepatitis (NASH) · K75.81 is a billable/specific ICD-10-CM code
  2. K76. 0 ‘Fatty (change of) liver (ICD 10)
  3. Nonalcoholic Fatty Liver Disease(NAFLD) K75.8 (ICD-10)

  6. Differential Diagnosis/Root Cause Analysis of Major Depressive Disorder DSM-5 296.20-296.36 (ICD-10-CM Multiple Codes)

Followup Tool If CLDQ Result is Moderate🟡 or High🔴

If results show that you have a moderate to high risk of progressive liver disease, then please calculate your Fibrosis 4 Index(also called Fib4 index) from our free Fib4 Calculator. This calculator needs you to enter data from your blood test report. If Your Fib4 score is >1.45, then you have a higher chance of progressing toward advanced-stage chronic liver disease.
